RESUMEN 
En este articulo, revisamos el diagnostico y tratamiento del estreñimiento funcional en los niños, no abordamos el estreñimiento orgánico. La diicultad en la defecación es un problema frecuente en la consulta de pediatría y supone el 25-45% de las consultas de los especialistas en gastroenterología pediátrica. En la mayoría de los casos es funcional, sin embargo el pediatra debe tener presentes siempre los síntomas y/o signos que puedan indicar la presencia de causas, inhabituales pero serias, de estreñimiento orgánico. Estudios recientes demuestran que las recomendaciones emitidas por las guías clínicas disponibles están basadas en la experiencia personal y en consensos más que en la evidencia cientíica. Las medidas terapéuticas incluyen desimpactación y tratamiento de mantenimiento consistente en cambios dietéticos, modiicación de los hábitos higiénicos y el uso de laxantes. La adición de ibra en la dieta puede ayudar. La mayoría de los niños mejoran con estas medidas, que con frecuencia deben mantenerse durante varios meses y solamente el 50-70% de los niños con estreñimiento funcional consiguen una mejoría a largo plazo.

MISSION 
The mission of the continuity experience is to help residents acquire
competencies essential for comprehensive, coordinated longitudinal care
of children with a wide variety of medical, behavioral, and social
problems.
To accomplish this mission, residents must be offered a structured
educational experience where they provide general pediatric care to a panel
of patients throughout residency training.  Such care must include routine
well child examinations, management of acute and chronic medical
problems, anticipatory guidance, preventive health care, management of
physical, psychosocial, developmental, and environmental problems.
To acquire competency in coordinated, longitudinal patient care, residents
should see patients in an efficiently run medical clinic or private practice
office setting.  Quality office practice management therefore is integral to
meeting the mission of the continuity experience.  Certain professional
attitudes, skills, and behaviors are especially important for coordinated,
longitudinal care of children.  The continuity experience must focus on
acquisition of these broad professional competencies to fulfill the intended
mission.

Introduction 
Combined Internal Medicine and Pediatrics (Med-Peds) residency training is a
unique and exciting way to become a  well-trained physician.  As program
directors, we find students eager for more information about the nature and
possibilities of such training.   In compiling this manual, our goal was to provide
up-to-date information about the history, mechanics, training, and career
opportunities available to the future Med-Peds physician.
As with any decision, careful consideration of your personal goals is the
foundation of the process.  The selection of a specific residency program happens
only after a well-researched decision on what specialty to pursue, followed by
examination of individual program strengths and weaknesses.  This is a process in
evolution; each encounter during training is an opportunity to focus on how best
to achieve your goals.
Good decisions get better with information from the best sources, and this guide
is meant to assist in that process.  Communicating with those who have already
followed a given path is important, and we strongly encourage you to seek out
Med-Peds physicians in your community.  There is no substitute for the expertise
of one who has already been through residency and successfully practices his or
her art every day.
